">Responsibilities
">In this role, you will be responsible for coordinating patient care delivery, consulting with other departmental staff, completing documentation, scheduling family conferences, and coordinating weekly patient care team conferences.
">Requirements
">You must have current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist. Certification in Case Management or Rehabilitation Nursing is preferred. A minimum of 2 years social work or case management experience in an inpatient setting is highly preferred.
