Hours & Benefits:
- Work Schedule: 40 hours per week – Tuesday to Saturday (8:30 AM – 5:00 PM) or Sunday to Thursday (8:30 AM – 5:00 PM)

Requirements:
- High School Diploma or equivalent
- Valid NYS Driver’s License (required for most positions)
- Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check
Key Responsibilities:
As a Case Coordinator for our Ulster Treatment Apartment Program (ULTAP), you will:
- Manage a Designated Caseload: Ensure the accurate and timely completion of all case documentation in the Agency’s electronic health record system.
- Build Therapeutic Relationships: Establish and maintain a strong rapport with clients, encouraging and supporting them in meeting their personal service plan goals.
- Teach & Evaluate Daily Living Skills: Help clients develop and improve essential daily living skills, offering guidance and monitoring their progress.
- Empower Clients’ Decision-Making: Promote client autonomy by encouraging independent decision-making.
- Advocate for Clients: Provide support with benefits applications, medical and clinical services, and assist with treatment-related needs.
- Attend Meetings & Reviews: Participate in treatment and medication reviews, working collaboratively with clinical providers and clients.
- Coordinate Medical & Mental Health Care: Monitor and support clients’ medical and mental health needs, ensuring consistent care.
- Provide Crisis Intervention: Offer assistance as necessary in times of crisis, ensuring client safety and well-being.
- Ensure Compliance with HIPAA: Maintain confidentiality and protect clients' civil liberties, rights, and property.
- Supervise Self-Medication: Oversee clients’ self-medication processes and conduct weekly medication rotation runs.
- Conduct Site Visits: Ensure housekeeping standards are met and monitor daily living skills during scheduled site visits.
- Transport Clients: Ensure clients are transported to necessary appointments and services on time.
- Facilitate Education & Support: Conduct relapse prevention groups, MICA education, and provide random drug screenings as required.
- Collaborate Across Teams: Work with internal and external teams, including family members, behavioral health providers, and other collaterals.
- Engage Clients in the Community: Facilitate participation in community activities and assist with moving clients in and out of apartments when necessary.
- Attend Meetings: Participate in scheduled staff and supervisory meetings, contributing to the ongoing development of the program.
- On-Call Responsibilities: Provide on-call support as needed to ensure continuous care.
