What are the responsibilities and job description for the HOME CARE COORDINATOR position at RELIABLE CARE?
The role of the Home Care Coordinator involves working closely with clients and their families to assess needs, work on care plans and coordinate services. The duties includes amongst others; obtain care needs, ensure quality of care, maintain client satisfaction etc.
Experience:
Excellent interpersonal skills with the ability to communicate effectively with clients, families, and healthcare professionals.
A strong background in caregiving or direct support roles within home community care setting is a plus.
Job Types: Full-time, Part-time
Pay: $22.05 - $28.00 per hour
Expected hours: No less than 22 per week
