Residential Healthcare Specialist- $22.68

PT 40 Hr/wk Provides coordination and expertise related to program participant’s health needs. Acts as liaison between community-based providers, the agency nurse consultant, the team of CRS staff and the program members to help ensure member related medical information is effectively communicated and implemented. Responsibilities include, providing regular oversight of each CRS program members’ medication storage system to ensure necessary supply of medications, proper storage of medications, medication binder face sheet accuracy, medication self-administration acknowledgement accuracy, and overall organization of the members storage system.

Hours: Monday-Friday, 7:00am-3:00pm
Duties and Responsibilities:
- Act as liaison between community-based providers, the agency nurse consultant, the team of CRS staff and the program members to help ensure member related medical information is effectively communicated and implemented.
- Serve as programmatic point of contact for each program member’s pharmacy of choice to help ensure timely and accurate delivery of all medications.
- Serve as programmatic point of contact for each program members community based medical providers to help ensure that all relevant provider treatment plans are understood and communicated within the residential program.
- Collaborate with the team of staff and the nurse consultant to coordinate health care schedules towards most desirable outcomes for program members.
- Maintain an accurate and up to date record of all medical appointment results in Sharepoint, to be reviewed with RPD, Nurse Consultant and Service Director on a minimum monthly basis.
- Maintain a comprehensive timeline of each CRS program members’ medication changes in Sharepoint, including the date of the medication change, the prescriber, the previous prescription, the new prescription, and the reasons for the change.
- Maintain an accurate record of each CRS program members’ medication chart in Therap to help ensure accurate communication across community-based provider networks.
- Collaborate with Residential Program Director, Service Director and Nurse Consultant to help ensure that all medical related documentation practices are implemented at best practice standards in Therap.
- Provide regular oversight of each CRS program members’ medication storage system to ensure necessary supply of medications, proper storage of medications, medication binder face sheet accuracy, medication self-administration acknowledgement accuracy, and overall organization of the members storage system.
- Collaborate with Residential Program Director, Service Director and Nurse consultant to help ensure that all in-home systems of support related to each members’ medical status are clearly communicated to the team of direct care staff.
Education and Experience:
- Bachelor's Degree in a related field or equivalent work experience
- High School Diploma or equivalent
- Work experience in the provision of mental health services preferred
- Valid CT Driver’s License, vehicle registration and insurance
- Good driving record
