Summary
The Medical Coordinator is primarily responsible for the coordination of residential client’s medical and dental appointments, transportation to appointments and supervision of clients at appointments, preparation of medications to be sent to the homes, and medication management training. The Medical Coordinator has the responsibility to ensure that medication validations occur before medications are sent to the homes and has the primary responsibility for training new employees in medication management. A Medical Coordinator will also at times function in the role of residential counselor in the homes at which time they are responsible for the direct care of the clients.
Essential duties and responsibilities in the Medical Coordinator role include the following:
· Provide transportation for the client/consumer to and from activities/appointments.
· Train new employees in Medication Administration
· Must be available and flexible to accommodate the unexpected personnel need that arises.
(i.e. covering house shifts, emergency relief, vacations, office work and recreational needs.)
· Provide transportation for the clients to and from activities.
· Interact professionally with all medical, court and educational personnel.
· Communicate, coordinate, and plan daily schedule with all necessary departments.
· Create, track, and record medical and dental appointments for clients and consumers.
· Pick up prescriptions, perform medication verification and deliver to house staff
· All other duties as assigned by Program Manager
· Create Medication Administration Records and other medical/medication paperwork
· Track medication refill dates and reorder medications before expiration
· File Medical information in client/consumer charts
Essential duties and responsibilities in the Residential Counselor role include the following:
· Maintain constant alertness and eyes on supervision of consumers at all times.
· Observe safety measures and model appropriate, responsible behavior.
· Implement trauma informed interventions according to the needs and services plan as defined by the treatment team.
· Assist the child in developing and reaching goals.
· Assist consumers in identifying and resolving social or other problems.
· Manage the activities of the consumers under his/her care.
· Assisting with and teaching Daily Living or Independent Living Skills.
· Ensure that the consumers clean and trained in cleanliness and hygiene.
· Ensure Title 22 Regulations and RVI Policy & Procedures are followed when administering and documenting medications.
· Prepare meals and provide daily nutrition for consumers as required.
· Participate with consumers in general housekeeping of residence.
· Provide transportation for the consumer to and from activities/appointments.
· Use verbal de-escalation skills to prevent a behavioral crisis or physical assault.
· Assist disabled consumers in eating, dressing, and toileting.
· Management of aggressive and/or assaultive behavior.
· Complete required paperwork of consumer’s daily activities and behavioral incidents.
· Inform the Program Facilitator of any unusual occurrences or concerns and document as required.
Participate with responsible parties and agencies in planning for the consumer’s needs.
· This job has no supervisory responsibilities.
