Tricare Home Health Services Inc. is seeking a Licensed Vocational Nurse (LVN) to join our dynamic office team. This role offers a unique opportunity to contribute to case management, patient coordination, and complianceensuring smooth daily operations in a well-established home health agency.
As we expand into telehealth services, this position will also play a role in supporting our new telehealth program, helping to improve patient engagement, monitor care remotely, and assist with virtual patient outreach.

Key Responsibilities
Case Management & Coordination Oversee patient care plans, ensure regulatory compliance, and facilitate seamless service transitions.
Daily Operations Support Assist with chart reviews, documentation, patient follow-ups, and provider communication.
Patient & Provider Liaison Act as a bridge between patients, families, and healthcare providers to ensure effective communication and high-quality care.
Regulatory Compliance & Quality Assurance Maintain agency standards, ensure accurate paperwork, and support home health initiatives.
Telehealth Support & Virtual Patient Monitoring Assist with patient outreach, follow-up calls, and remote health monitoring, ensuring patients receive timely care through our new telehealth program.
What Were Looking For
Active LVN License in Texas
Minimum 2 Years Of Experience (Home Health Experience Preferred)
Strong Organizational Skills Attention to detail, ability to multitask, and manage priorities effectively
Excellent Communication & Teamwork A positive, professional attitude is essential
Tech-Savvy or Willing to Learn Comfortable using telehealth platforms, EMR systems, and remote monitoring tools
Bilingual (Spanish) Preferred But Not Required
