Location : Tricare Home Health Services Inc. – San Antonio, TX (Medical Center)
Schedule : Monday – Friday, Full-Time (No nights or weekends!)
Are you an experienced and detail-oriented LVN looking for a stable, office-based role in home health?
Tricare Home Health Services Inc. is seeking a Licensed Vocational Nurse (LVN) to join our dynamic office team. This role offers a unique opportunity to contribute to case management, patient coordination, and compliance —ensuring smooth daily operations in a well-established home health agency.
As we expand into telehealth services , this position will also play a role in supporting our new telehealth program , helping to improve patient engagement, monitor care remotely, and assist with virtual patient outreach.

Key Responsibilities :
Case Management & Coordination – Oversee patient care plans, ensure regulatory compliance, and facilitate seamless service transitions.
Daily Operations Support – Assist with chart reviews, documentation, patient follow-ups, and provider communication .
Patient & Provider Liaison – Act as a bridge between patients, families, and healthcare providers to ensure effective communication and high-quality care.
Regulatory Compliance & Quality Assurance – Maintain agency standards, ensure accurate paperwork, and support home health initiatives.
Telehealth Support & Virtual Patient Monitoring – Assist with patient outreach, follow-up calls, and remote health monitoring , ensuring patients receive timely care through our new telehealth program .
What We’re Looking For :
Active LVN License in Texas
Minimum 2 Years of Experience (Home health experience preferred)
Strong Organizational Skills – Attention to detail, ability to multitask, and manage priorities effectively
Excellent Communication & Teamwork – A positive, professional attitude is essential
Tech-Savvy or Willing to Learn – Comfortable using telehealth platforms, EMR systems, and remote monitoring tools
Bilingual (Spanish) Preferred but not required
