ROLE PURPOSE
This position will be responsible for scheduling patient visits. Responsibilities include the following:
- Obtains daily report from clinicians regarding visits made/missed/moved.
- Manages therapy evaluations to ensure that reevaluation standards are met.
- Provide effective communication to patients, staff members and other health care professionals or referral sources.
- Runs schedule deviation report daily to identify scheduling issues.
- Verifies physician orders against the schedule to ensure frequency/duration accuracy.
- Respond to agency and patient needs in a professional manner.
- Schedule patient visits in the EMR in a timely manner.
QUALIFICATIONS
- Home Health experience preferred
- Wellsky experience preferred
