We are seeking a highly skilled RN to join our Hospice Admissions team. In this role, you will provide direct care to patients and their families, coordinating with the interdisciplinary team to ensure an optimal care experience. Key Responsibilities:

• Assess patients' physical, emotional, and spiritual needs and develop personalized plans of care
• Provide education and support to patients and their families to promote self-care and independence
• Collaborate with the interdisciplinary team to ensure seamless coordination of care
• Maintain accurate and up-to-date clinical records and reports
• Participate in quality improvement initiatives to enhance the delivery of care

Requirements:

• Current RN licensure in the state of operation
• 1 year nursing experience required, 3 years preferred
• Experience in hospice or a similar setting preferred
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
