We are seeking a dedicated Clinical Care Coordinator who will primarily focus on scheduling and front desk duties, with a secondary focus on nursing tasks at our Public Safety Office supporting our first responder community. This role involves managing patient appointments,ensuring smooth communication between patients, providers, and other healthcare entities as well as administering vaccines. The ideal candidate will possess excellent organizational skills, attention to detail, and a passion for serving our community.
Responsibilities:
Care Coordination (50%):
- Manage patient referrals and chart creation.
- Retrieve, send, and maintain medical records.
- Order and maintain labs as required.
- Make follow-up calls to patients and other clinics.
- Ensure smooth communication between patients, providers, and third parties.
- Track patient health using Electronic Health Records (EHR) and case management tools.
- Attend weekly meetings to ensure efficient care coordination and proper case management.
Front Desk/Scheduling (30%):
- Greet patients and check them in and out.
- Answer and make calls to patients to schedule appointments.
- Schedule appointments and send confirmation emails.
- Coordinate with recruiters regarding applicant appointments.
- Ensure timely communication with patients regarding their appointment details.
Nursing with Vaccine Administration (20%):
- Administer vaccines and injections at various Fire/Police Department sites.
- Assist in vaccine tracking and documentation for reporting purposes.
- Provide patient education on vaccines and general health practices.
- Ensure proper infection control and vaccine administration procedures are followed.
Qualifications
- Valid LPN license OR Phlebotomy certification in addition to one year of related experience
- Experience in healthcare scheduling, billing and/or registration.

Environmental/Working Conditions:

- Requires hand dexterity for operating office equipment and a computer keyboard.
- May require stooping, bending, and sitting for extended periods.
- Ability to lift and transport items weighing up to 10 pounds.
