Job Summary:

The Outreach Case Management Coordinator is a dual role combining youth services and community engagement. They are responsible for providing mobile outreach to youth who are at-risk of or currently experiencing houselessness on O‘ahu while implementing assessments and data entry. Other responsibilities include partnering with the community to increase awareness and decrease stigma. Those with lived experience are strongly encouraged to apply!

Job Responsibilities:

A. Street Outreach

· Collecting and reporting data on outreach activities including services provided, system enrollment, number of individuals reached, and/or any barriers or challenges encountered through Homeless Management Information System (HMIS.)

· Training and supervising outreach team members regarding data entry, billing and case management duties.

· Utilizing outreach data statistics to support agency reporting and effective outreach strategies such as identifying target areas and demographics.

· Provide mobile outreach coordination to locate and develop rapport with youth who are at risk of houselessness or unsheltered.

· Consistent case management and care coordination across multiple departments and/or agencies including completion of assessments, treatment/housing/shelter referrals and linking youth to community resources.

· Complete Homeless Verification Letters, service assessments and maintain client files in HMIS to ensure youth are eligible for the Youth Coordinated Entry System (CES.)

· Additional case management duties include assisting youth with benefit applications (SNAP, GA, SSI, SSDI, etc.) and obtaining vital documents including State ID, birth certificate, and social security card.

· Demonstrate high integrity when dealing with a wide array of cultural, traumatic and confidential information.

B. Community Outreach

· Plan and coordinate outreach activities and community awareness events with partners.

· Support and partner closely with subrecipients and peer organizations.

· Develop and maintain positive relationships with schools, law enforcement, health & mental health providers, hospitals, substance use treatment centers, churches, and other community partners who provide services or referrals to the RYSE program.

· Attend community meetings with other service providers to share program information and coordinate services.

· Assist with other duties as it relates to at-risk and houseless issues facing youth as assigned.

· Maintaining and interacting in a culturally sensitive, respectful, and professional demeanor with youth, staff and community partners.

Required Qualifications:

· Strong reading, writing and computer skills including data entry and analysis.

· Ability to maintain regular, consistent attendance including non-traditional hours (afternoons, evenings, some weekends or holidays) as needed.

· Familiarity with O‘ahu geographical regions and ability to become familiar with available youth resources.

· Successful completion of Employment Eligibility Verification and background screening.

· Ability to obtain CPR/First Aid Training certification.

· Ability to drive with a clean recent drivers abstract.

Educational Requirements:

· Bachelor’s degree preferred, High School diploma or GED required.

· Lived experience may be considered in place of or in addition to formal education requirements.

While performing this job, this position typically spends time sitting, standing, climbing stairs, walking, driving, carrying (50lbs), and lifting (50lbs), bending, keeling, using a computer, firm/strong grasping, and repetitive hand control, makes and receives telephone calls, use of general office equipment copier, fax machine, and basic office equipment. All other duties assigned.

Work Environment

Outreach Case Managers work with youth in the field in all types of environments and housing situations. Job Type: Full-time

Pay: $22.00 - $24.00 per hour

Expected hours: 40 per week
