Job Requirements:
· High school diploma and/or GED; must be 18 or older
· Bachelor’s Degree in related field is preferred
· Valid driver’s license with dependable, insured vehicle
· Pass background screening
· Complete and pass all required trainings
· Ability to accurately complete required daily documentation
· Responsible, clean, professional
· Directly supervise a small team of direct support professionals (DSPs)
Purpose Statement:
Provide management and oversight to the DSP staff at the Excel day program. Provide assistance and habilitative services to the individual’s attending the Excel day program.
Responsibilities:
· Ensures the day-to-day operations of the day program.
· Serve as an agency executive liaison with all related federal, state, and local agencies; including but not limited to, DMH Regional office, TCM entities, guardians and individuals served
· Provide professional oversight, observation, assessment, and documentation of services
· Facilitate the planning, development and implementation of person-centered plans; develop individual goals and outcomes with supported individuals; coordinate and participate in individual plan meetings; monitor, document and report progress toward meeting outcomes; ensures that all needed supports are in place to meet outcomes; coordinates communication with other service agencies to ensure individual outcomes are met
· Assist the director with hiring, orientation, training and providing supervision to day program staff
· Complete staff reviews as stated in current agency policy
· Assist in scheduling, hosting and facilitating team meetings as needed to address concerns, provide updated changes, directives and policies.
· Create and implement staff schedules
· Set up and maintain the individual books in reference to licensing and agency regulations
· Establish and implement program goals, arrange or provide staff orientation and training, directly supervise staff, maintain a quality assurance program, providing an environment that is conducive to the welfare of the service recipients and is in accordance with federal, state, local and organizational regulations
· Assist in tracking and maintaining compliance with staff training records
· Manage and maintain staff ratio within the program.
· Ensures program is fiscally responsible, follow policies and procedures for procurement. Ensure financial records are complete, thorough and submitted in a timely manner. Complete audits of program and consumer finances to ensure monies are safeguarded
· Develop and cultivate positive relationships with individuals, families and other stakeholders ensuring continued effective communication with said individuals, families and stakeholders
· Assures frequent communication with Program Director, Central Office, HR and other RH personnel as needed.
· Provide direct care to program individuals and implementation of ISP and behavior plans
· Assist individuals in the pursuit of recreational, social and community activities based on their preferences and individual plan
· Document program data in Therap daily: including but not limited to individual and behavioral plan data, community activities, individual funds spent etc.
· Treat all individuals and coworkers with dignity, respect, and fairness; provide support to other team members when individuals are having behavioral issues
· Maintains client and staff confidentiality
· Other duties may be assigned
Job Type: Full-time
Pay: $36,000.00 - $45,000.00 per year
