- Valid driver’s license with dependable, insured vehicle
- Pass background screening
- Complete and pass all required trainings
- Ability to accurately complete required daily documentation
- Responsible, clean, professional
Purpose Statement:
Provide assistance and support to each individual’s daily activities including personal growth toward identified individual goals, personal care, medication administration, and community involvement.
Reports to:
Excel Day Program Coordinator
Responsibilities:
- Provide direct care to program individuals and implementation of ISP and behavior plans
- Teach daily skills such as self-help, socialization, community interaction and advocacy, as appropriate according to each individual plan
- Assist and encourage individuals to care for immediate personal needs such as toileting, hand washing, eating etc.
- Administer medications to individuals during program hours and observe for any changes in behavior or health that may have resulted from mediation administration
- Assist individuals in the pursuit of recreational, social and community activities based on their preferences and individual plan
- Promote participation and involvement in community activities
- Provide transportation to and from community activities during program hours
- Document program data in Therap daily: including but not limited to individual and behavioral plan data, community activities, individual funds spent etc.
- Document individual’s health as required (i.e. seizure log, bowel movements, temperature)
- Document medication administered during program hours on individual MAR
- Ensure all documentation is accurate and completed at the end of each shift
- Observe individuals for evidence of injury or bruises and evaluate for changes in emotional or physical status
- Report concerning and/or medical related incidents to the Excel program director immediately and complete a written incident form
- Ensure each individuals’ rights are protected
- Treat individuals with dignity and respect and encourage positive peer interactions
- Maintain client confidentiality
- Perform all duties in accordance with State of Missouri regulations and Restoring Hope’s policies and procedures
- Complete and maintain all required State mandated trainings such as Medication Aide, CPR/First Aid, and all other trainings required by Restoring Hope
- Attend meetings and training sessions as required by Restoring Hope and complete annual trainings related to individual’s ISP and related plans
- Work all assigned schedules
- Includes – but not limited to arriving and departing on time, requesting time off in advance (4hr notification for Call-Ins and/or Shift Changes), notifying supervisor of changes to your assigned work schedule promptly
- All Shift Changes must be approved by Excel program director
- Treat all individuals and coworkers with dignity, respect, and fairness; provide support to other team members when individuals are having behavioral issues
- Act as a representative for Restoring Hope in a professional manner across all settings; families, service coordinators, State officials, community partners, local businesses, and employers
- Serve as an active member of the support team
- Create/locate curriculum and educational materials for active daily learning
- Develop and implement new activities/ideas
- Develop relationships within the community for volunteer and community partnership opportunities
- Other duties may be assigned

Pay: $15.00 - $17.00 per hour
Expected hours: 40 per week

- No weekends
