  • BLS EPIC experience a must
  • On Call requirement for overnight and weekends equates to 2-3 times a week
  • Prior experience preferred

  • Prime OIG
  • Prime Drug Screening (minimum 10-panel test) - Within 60 days of start
  • Prime Color-blind test - Must be completed annually
  • Prime Agency Attestation
  • Prime SAM / EPLS
  • Prime Resume
  • Prime Reference #1
  • Prime Reference #2
  • Name Verification Affidavit - Please upload a notarized name affidavit for any name discrepancies between credentials
  • COVID-19 Vaccine - 2 vaccines and booster required or declination completed
  • MMR - Proof of 2 Vaccines or Positive Titer showing immunity
  • Fit Test - Annual Respirator fit test
  • Hepatitis B - Declinations not accepted at Shasta Regional Medical Center
  • Influenza Vaccine - Required October 1-May 31
  • TB Test - Required
  • Varicella - Vaccination / titer showing immunity
  • Tdap - Declinations not accepted at Shasta Regional Medical Center
  • Annual TB Questionnaire - Required
  • Positive PPD- Negative Chest X-Ray - Optional
  • Social Security Card
  • Physical / Health Statement - Annually, Except Saint Marys Regional Medical Center- Nevada must be completed within 60 days of start
  • Prime Core Competencies Exam - Must score 80% or higher
  • Prime Criminal Background Check - National Criminal Background Check within one year of start date
  • Prime Badge Photo - JPEG only photo
  • Prime Form I-9 (Employment eligibility verification) - Agency attestation accepted
  • Prime Job Description - Signed by employee
  • Prime COA
  • DCI / BVP - Certification required
