Is responsible for the coordination of patient visits, maintenance and upkeep of scheduling records, and logbooks.
- Staffing coordination duties include the accurate, and timely, communication of scheduling changes between office and field staff.
- Maintain a current patient roster, with necessary information.
- Assist in coordinating services provided to patients.
- Control and monitor schedule changes.
- Check compliance of visits done (on master schedule), after all notes are matched to charge; bring any scheduling problems to the supervisor's immediate attention.
- Assist in taking referrals, makes copies of referral sheets for Payroll/Billing Manager, and on-call coordinator, when requested.
- Call hospital Social Services department to notify them that an Agency patient has been admitted to their hospital.
- Call case managers/social services to verify correct demographics on a patient's face sheet, including Medicare/Private Insurance number and correct primary care physician and contact information
- Obtain weekly visit count and report results to the Director of Nursing and Director of Operations
- Assist in relaying messages to field staff, office staff, and community liaisons; give, and take, reports from the on-call coordinator.
- Coordinate patient care, among disciplines.
- Confer with Director of Nursing on problems and complaints.
- Perform all coordinating duties.
- Review, and direct, case management notes, document change of Aide, change in service, telephone complaints, etc.
- Supervise the completion of intake forms, and their appropriate routing, for admission approval.
- Supervise the scheduling of appropriately skilled person to fill the case, the input of the scheduling information on the computer, and continue to supervise the coordination of scheduling.
- Supervise the scheduling of replacement workers, as needed, and the input of the scheduling information.
- Communicate with patients/families on Home Health Aide-related issues.
- Take telephone referral information.
- Supervise the maintenance of case management notes, related to personal problems and/ other situations that arise with staff, including those relating to patient care.
Education:
- Minimum: High School diploma or GED
- Preferred: Bachelor's Degree
- Minimum: At least 2 years of experience in the home health industry
- Preferred: At least 5 years of experience in the home health industry
- Preferred: LPN/PTA/OT
- Strong organizational and time management skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced and dynamic environment.
- Proficient in Microsoft Office Suite and patient management software.
- Knowledge of healthcare regulations and policies
- Ability to sit and/or stand for 2 hours.
- Ability to type at a computer for 6 hours.
- Ability to lift 35 lbs.
