Position Summary
The Housing Tenancy Case Manager provides direct services to individuals who have been referred to CalAIM Community Supports Housing Tenancy services. The Housing Tenancy Case Manager is responsible for providing supportive services to clients while in housing to support clients in remaining housed. Under the direction of the CalAIM Program Coordinator, the Housing Tenancy Case Manager is responsible for creating a care plan that meets the requirements of the health plans and addresses barriers to each client remaining housed. Services may include, but are not limited to assisting clients with increasing income, acting as a liaison with property management, assisting with annual housing documentation, and assisting clients with developing skills to upkeep their housing.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Serve as the Housing Tenancy Case Manager for clients enrolled in CalAIM CS
- Maintain appropriate client files and complete documentation within the required timeframe
- Advocate for client’s voice and choice in housing process
- Support client’s in increasing income through resume building, practice interviews, and job search
- Assist clients with meeting all basic needs including locating food resources and applying for entitlements
- Assist clients with housing related paperwork such as annual recertification
- Coach clients in learning and developing daily living skills needed to maintain housing
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelors Degree in social services or closely related field, preferred
- 3 years experience in housing related social services
- Cleaning driving record
