The MnCHOICES Certified Assessor completes assessments and provides case management services to individuals under the age of 65 in Rice County regardless of payer source. Assessments provide information and resources to individuals and/or their legal representatives based on their stated preferences and assessed needs. Assessments are also used to determine an individual's eligibility for Long Term Services and Supports, and the Minnesota Waiver Programs.
Minimum Qualifications: This position requires a Bachelor’s degree in Social Work or related human services field, and two years of supervised experience working with individuals with physical disabilities, mental illness, brain injuries or developmental disabilities. A valid Driver’s license is required upon hire. The Social Worker must be a MnCHOICES Certified Assessor through the State of Minnesota, ability to complete upon hire.
Preferred Qualifications: A Master’s degree in Social Work, and three years of experience as a County Social Worker is preferred.HIRING RANGE / UNION: $34.79 - $36.37 per hour / SS unit - Grade 12
HOURS: Business hours are 8:00am - 4:30pm, Monday - Friday. Full-time hours equal 40 hours per week.

- Completes MnCHOICES assessments for individuals to identify appropriate options for care.
- Provides case management services to individuals under the age of 65 residing in Rice County receiving Community First Services and Supports (CFSS) services, and not on a Minnesota Waiver Program.
- Attends training sessions and staff/community meetings.
- Completes intakes for assessment request from individuals and/or community partners.
- Performs other duties as assigned.
