Minimum Qualifications & Abilities:
Essential Functions:
- B.A./B.S. and/or three years of relevant experience in one or more major student and/or family support areas.
- Experience in specific work-related field may serve in lieu of formal education requirement.
- Experience in a public agency, government-funded or similar program; experience in a health, education, or social service related field preferable
- Ability to establish and maintain effective collaborative relationships with staff, community partners, and local and state agencies
- Experience with the development of evidence-based prevention and intervention
- Ability to work with individuals of diverse backgrounds
- Knowledge of community agencies and resources
- Valid driver’s license with acceptance DMV printout
Essential Functions:
- Works collaboratively with families, community-based organizations, government entities, and school administrators
- Provides outreach and oversight and coordination of school and community-based services and service providers
- Prepares, plans, develops, and facilitates professional learning opportunities for community school stakeholders
- Coordinates, develops, and facilitates community meetings to gather information about the needs of the students and families
- Facilitates the cross-referral of students and families between service providers
- Conducts ongoing needs assessments in order to identify gaps in programs, services, and supports as well as capacity and assets.
- Attends meetings, conferences, and trainings required for the program
- Identifies and develops school site resources for gaps in service areas
- Supports the implementation of district-wide school climate goals
- Develops and/or supports data collection
- Coordinates linkage of identified students and families to appropriate programs, activities, and services
- Conducts home visits to support students and families in achieving student success
- Organizes parent engagement workshops, including logistics and communication
- Compiles data, materials and creates files for reporting program goals and outcomes
- Prepares notifications such as brochures, pamphlets, newsletters, online publications, and social media posts to communicate announcements to the community and to publicize the resources of the school sites/programs
- Contacts external agencies and vendors to obtain or disseminate information on outreach opportunities and community events as well as plan logistics and accommodations for sponsored events
- Prepares and presents information sessions to community organizations, administrators, school staff, parents, and the public to raise awareness of program.
- Contributes to grant applications and reports as needed.
- Maintains the community calendar of events to inform students and families, school staff, and other community organizations of outreach activities.
- Performs other duties as assigned by Superintendent or designee.
Salary : $77,547 - $94,257
