Job Summary
The Case Manager plays a vital role in supporting our consumers and families with providing services to our adults with Intellectual Disabilities. This position involves assessing consumer needs, gathering data, developing personalized care plans, and providing ongoing support to ensure consumers achieve their goals. The ideal candidate will have a medical/clinical background, possess strong interpersonal skills, attention to detail and the ability to work with various support team members.
Responsibilities
- Complete and coordinate the Personal Outcome Measures assessment, utilizing this information to compile the annual summary and implementation plans.
- Provide ongoing case management services, including intakes, managing regular check-ins and documenting progress evaluations.
- Supervise and mentor consumers through crisis management situations, ensuring their safety and well-being.
- Maintain accurate documentation of consumer interactions, Doctor appointments, progress notes, and service referrals.
- Other duties needed to prepare and present documentation needed to further evaluate or request additional services.
Qualifications
- Excellent communication skills, both verbal and written.
- Proficiency in mentoring and supervising individuals from diverse backgrounds.
- Candidate must be organized, and possess strong interpersonal skills.
- Able to work independently as well as part of a team-oriented environment.
- Strong crisis management skills with the ability to remain calm under pressure.
- Commitment to maintaining confidentiality and ethical standards in all consumer interactions.
- Previous experience working with adults with intellectual disabilities in a medical or clinical field a plus
- Prior work experience to understand medical terms and diagnosis' helpful.
