Seeking a skilled mental health paraprofessional. The Integration Specialist is providing Psychiatric Rehabilitation services to aid the consumer by teaching, modeling behavior, and assisting with the development of crucial coping skills.
Job requirements of Integration Specialist, include but are not limited to:
- Conduct regularly scheduled consumer visits
- Provide identified PRP rehabilitation and support services in accordance with the consumer's Individual Rehabilitation Plan (IRP)
- Maintain required and consistent documentation for each client, including contact/progress notes, monthly summaries, and quarterly reviews
- Participate in ongoing trainings and supervisions
- Maintaining a client caseload of clients
- Work from a Client-Centered Approach
- Understand the importance of self-determination in meeting a client’s needs
- Determine if the client has, or maybe eligible, for Federal or State entitlements
- Assist the client, if necessary, to apply for all entitlements for which the client may be eligible if they do not have entitlements
- Encourage community integration and access to local support services
- Goal setting and treatment plan development
- Asses the client’s strengths, needs, abilities, and progress
- Provide supportive services that encourage independence
Qualifications
- Experience in mental health field
- Strong communication and interpersonal skills
