Position Title: Community Living Support Manager
Supervised by: Director of Residential Programs
Position Summary:
As Community Living Support Manager, you will ensure that people living with disabilities receive the absolute best support possible and supervise a team of drivers that meet our clients’ transportation needs. Reporting to the Director of Residential Programs. The Community Living Support Manager helps implement individual personal support plans (ISP) and behavior plans (BP). The Community Living Support Manager must perform all duties in accordance with State of Oregon regulations and Rise & Shine policies and procedures.
Duties and Responsibilities:
- Manage a team of drivers dedicated to the Community Living Support program.
- Create, schedule, and organize enriching and engaging activities for clients to take part in.
- Build community partnerships that expand Rise and Shine’s network of professionals and organizations with shared missions and goals.
- Document program data, including but not limited to personal support plan data, behavior plan data, community integration logs, assessments, individual funds requests, mileage logs, maintenance requests forms, and supply acquisition forms.
- Responsible for completing and maintaining all required Oregon State mandated trainings such as Medication Administration and CPR/First Aid.
- Attend meetings and training session as required by Director of Residential Programs; completed annual training related to the individuals Personal Support Plans, Behavioral Plans.
- Provide support when individuals are having behavioral issues.
- Ensure all documentation is completed accurately and in a timely manner.
- Ensures Medication Administration Record (MAR) entries are completed Daily.
- Maintain OIS/CPR (Rise & Shine to provide training) certification.
- Gather, collaborate, and analyze data/information to develop Functional Behavior Assessments and Behavior Support Plans.
- Maintain company vehicles in safe and operating condition. Schedule preventative maintenance and monitor and maintain vehicle tracking software.
- Plan, organize, and facilitate company parties and events for clients and staff members.
- Other duties as assigned.
Minimum Qualifications:
- Oregon Driver’s License to drive our company vehicles.
- A bachelor’s degree or an equivalent work experience of at least 2 years in management.
- Master of Education or specialization in special education is preferred.
- Industry experience supporting individuals experiencing disabilities,
- Must be able to lift to 40 lbs., bend, and/or squat.
- Must be able to walk up and down stairs.
- Must be able to perform the job with or without reasonable accommodation.
- Pass a state-mandated background check.
