Position Summary
The purpose of this position is to supervise the daily operation of a Day Training & Habilitation (DTH) program that provides life enrichment opportunities for people with medical, physical and cognitive challenges.
Responsibilities
- Supervision, support and evaluation of activities including oversight of staff.
- Oversight of the license holder's responsibilities assigned in the person's CSSP Addendum and Individual Abuse Prevention Plan.
- Instruction and assistance to staff in implementing outcomes.
- Recruits, hires and supervises team of direct support professionals.
- Coordinates and provides staff backup supports to assure continuity of services.
- Assures performance of service team meets CARF regulatory standards.
- Maintains excellent communication with all Rise staff, and members of participants interdisciplinary team.
Requirements
- 3 years of supervisory level experience in a program providing direct support services is required.
- A degree in a human service related field and experience providing direct service to individuals with significant disabilities, preferably in a DTH setting.
- Must meet the qualifications required of MN 245D statute's Designated Coordinator role.
- Ability to direct and motivate others, perseverance and compassion for team members and the people we serve.
- Proficient with Microsoft Office software (Outlook, Word, etc.) and interest to learn and become proficient in case management software system (Care Director).
- Current, valid driver's license and driving record that demonstrates safe, legal driving habits.
