ESSENTIAL DUTIES AND RESPONSIBILITIESRiseBoro Community Partnership’s Wellness Rising program is an ambitious initiative partially funded by DOHMH to promote Health and Wellness education and address health disparities in Brooklyn. The Community Health Worker Supervisor is responsible for supervising the activities of Community Health Workers (CHWs) and providing them with coaching and support to ensure the quality of their work and accurate monthly reports. Specific duties and responsibilities include : Provide overall supervision of community-based activitiesEnsure a high quality and timely implementation of community-based activities by the CHWsBuild and maintain strong cooperation with CHWs to keep them informed about community health activitiesBuild strong partnerships with local health facilities and ensure necessary training takes place in the catchment areaEnsure safe and accurate use of all assistive medical devices i.e., blood pressure monitorsCreate and coordinate the work schedules for all CHWsEstablish a monthly work plan and calendar of activities to share with CHWsConduct weekly on-site supervision meetings to help support CHWsMeet with CHWs weekly to review program success and challengesCollect and input activity report data into the funder portal / information systemEnsure the quality and accuracy of all reportsReview all presentations on a variety of h ealth-related subjects before community eventsRemain updated and informed on all health-related material that will be shared during presentationsUpdate all materials with new information provided by the CDC or NYDOHMH concerning health alertsMaintain accurate daily logs, case notes, and files for all participantsAttend scheduled case review meetings, staff meetings, and assigned / required trainin gOther relevant duties as assignedCOMPETENCIESDemonstrated supervisory experience requiredHighly Proficient in English / Spanish; ability to read, and write in English and Spanish requiredAbility to troubleshoot and problem-solve proactivelyExperience in community engagement, campaign work, or other forms of direct outreach preferredExperience working in underserved communitie s with a genuine interest in improving community health preferredExcellent organizational skills and attention to detailStrong communication skills with the ability to speak comfortably and courteously to a diverse population, while maintaining a professional and warm demeanorExcellent interpersonal skills and customer service oriented; ability to maintain positive relationshipsPossess an entrepreneurial spirit and a positive attitudeAbility to ability to accept and follow directions; verbal and written instructionsFamiliarity with Bushwick, Brooklyn, NY area preferredAbility to travel to multiple locations in the community with necessary work supplies and materialsAbility to lift or carry up to 20lbsValid NYS license preferredAbility to be adaptable and flexible with assigned work schedule; evenings and weekends as requiredAbility to exercise good judgment and maintain confidentiality, particularly with client informationProficient in Microsoft Office suite, intermediate knowledge in Excel and WordCommitment to RiseBoro’s mission, vision, and valuesAbility to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communitiesDemonstrated commitment to supporting communities that have experienced systemic oppression and bias (i.e., people of color, LGBTQ people, immigrants, justice-involved persons, etc.)EDUCATION AND EXPERIENCEHigh School Diploma or equivalent (HSE / GED) requiredCommunity Health Worker Certification preferredA minimum of two (2) years of supervisory experience preferredQualificationsIt is the policy of RiseBoro to provide equal employment opportunity to all employees and applicants for employment.
