River Bend Nursing and Rehab is seeking a dedicated and compassionate Social Services Director to join our team. This full-time position plays a vital role in supporting the social and emotional well-being of our residents while ensuring the effective operation of the Social Services Department.
Key Responsibilities
- Develop comprehensive social assessments and care plans addressing residents’ social and emotional needs, with actionable and realistic goals.
- Actively participate in pre-admission and admission processes to ensure seamless transitions for residents and families.
- Provide empathetic support to residents and families during periods of adjustment, crisis, or special circumstances.
- Maintain accurate documentation of residents' behavioral and psycho-social statuses.
- Facilitate individual and group counseling sessions to meet residents’ needs.
- Establish and grow volunteer programs tailored to enhance social services initiatives.
- Orient new residents and their families to our facility, promoting a welcoming and supportive environment.
