We are looking for a individual who is eager to help serve within their community as a Housing & Benefits Coordinator for our Flexible Integrated Treatment (FIT) program located in North Sacramento.

Program Summary:
FIT serves infants, children and youth (ages 0-21) with a team approach to treatment planning allowing clinicians the flexibility to assess the level and type of services needed. As treatment needs vary over time, services are tailored to fit without interruption. Equally flexible is the location of service: home, school, River Oak clinical sites or community setting to best accommodate the family/care provider. A wide array of treatment interventions, to include multiple evidence-based programs, are used to include individual, family and group therapy.
Job Responsibilities:
- Develop a housing/permanency plan for each child or youth served.
- Assist youth and family members in developing and managing personal budgets through housing and budget counseling sessions, including establishing payment plans for bills and past debts.
- Assist youth and family members in locating and securing housing of their choice.
- Develop community collaborations and connections with community property owners/landlords, sober living homes and other service providers to assist in identifying and assisting youth and family members in gaining access to benefits, education, and/or housing.
- Assist youth and family members in the completion of applications/paperwork for SSI, Medi-Cal, and other benefits, Section 8, Job Corps, CA Conservation Corps, Military, education, and housing programs.
- Assist youth and family members in gaining access to benefit programs such as Cal Works, Department of Rehabilitation, etc.
- Conduct home visits with youth and family members to ensure stability and progress towards treatment goals and housing plan goals.
- Act as a liaison to community and the Flexible Integrated Treatment (FIT) Team by facilitating cooperative relationships with other social service agencies and representing River Oak or agency to the public.
- Documentation of services must be timely and accurate to ensure continuity of care and eligibility for reimbursement of services.
- Establishes and maintains a cooperative working relationship with all members of the agency.
Minimum Qualifications:
- Must have successfully completed twelfth grade (or the equivalent of completing twelfth grade)
- PLUS at least one (1) year of experience in the areas of community resourcing, housing resourcing, vocational/occupational assistance, educational rights and advocacy, connection with cultural communities, etc.
- Prior work experience or lived experience in foster care, juvenile justice, and/or with Rapid Rehousing and Homeless Prevention Programs highly desired.
- Must have excellent organizational, relationship, behavioral, and communications skills.
- Must be able to work in a team environment, in the office and in the general community, in a positive and safe manner, engage and build positive relationships with service providers, prioritize responsibilities, and build rapport with a wide variety of individuals.
