Riverbank Post Acute is a 99-bed Medicare and Medi-Cal certified skilled nursing facility and is one of the newest additions to the American Specialty Management group based in Modesto, California.

Coordinate and manage the admission process for new residents, ensuring all necessary paperwork and documentation are completed accurately and in a timely manner.Collect and verify resident information, including medical history, insurance coverage, and personal details, in compliance with facility policies and regulations.
Collaborate with nursing staff and other departments to gather necessary information and ensure a seamless transition for new residents.
Serve as the primary point of contact for prospective and newly admitted residents, answering inquiries, providing information, and addressing concerns promptly and professionally.
Conduct facility tours for potential residents and their families, showcasing the amenities and services available and highlighting the unique features of our skilled nursing facility.
Assist residents and their families in completing necessary forms and understanding the admission process, ensuring a positive and welcoming experience.
Maintain accurate and up-to-date resident records, both physical and electronic, including admission forms, insurance information, consent forms, and other relevant documents.
Ensure compliance with legal and regulatory requirements by adhering to confidentiality and privacy protocols for resident records and sensitive information.
Prepare and maintain reports related to admissions, occupancy rates, and other relevant metrics, providing regular updates to management.
Provide general administrative support to the admissions department, including answering phone calls, scheduling appointments, managing calendars, and handling correspondence.
Collaborate with other administrative staff to ensure efficient office operations, such as ordering supplies, maintaining inventory, and organizing files and documents.
QUALIFICATIONS/SPECIFIC REQUIREMENTS:
EDUCATION:
Must possess, as a minimum, a High School Diploma.
Bachelor’s Degree is preferred.
EXPERIENCE: Must possess, as a minimum, 1 year of relevant experience.
LICENSE: Vocational Nurse License
