Outreach Case Manager
Job Summary-The Case Manager is responsible for all intake, assessment, treatment planning, and progress note documentation for each client assigned to their caseload. Case Managers work collaboratively with the Program Manager to provide the most appropriate addiction services to match the client’s level of motivation and preference for ongoing recovery.
Responsibilities
1. Engage individuals with indicators of substance use disorder (SUD) and co-occurring disorders as they present through outreach or identification of collaterals and stakeholders.
2. Provide information and access to recovery resources including, but not limited to: warm hand-off referrals to treatment across the ASAM continuum of care, behavioral health providers, medical providers, prescribers for medication-assisted treatment (MAT) and psychiatric medications, self-help services, peer-recovery services, job training, cash assistance, assistance with family reunification, and any other client identified recovery needs.
3. Representing the expertise of recovery-based services and resources, engage and coordinate care with collaterals and stakeholders who may be: recovery support providers, behavioral health providers, family services, housing advocates, medical providers, lawyers, probation officers, clergy, Department of Transitional Assistance (DTA).
4. Responsible for accurate and timely documentation of goals, objectives, and progress notes developed with the individuals served based on their identified needs.
5. Engage in recurring individual or group supervision with Riverbend's Clinical and Outreach Supervisors.
6. Understand the treatment continuum and act as a liaison between the individual and treatment providers.
7. Knowledge of identified community resources as they relate to SUD and facilitate the transfer of care to the appropriate treatment or recovery service, treatment provider, and/or recovery support in the community.
8. Basic understanding of informed consent, HIPAA and confidentiality laws, and protocols around obtaining this consent to provide necessary follow-up services.
9. Develop and implement a safety response plan as needed.
10. Use consultation from the multidisciplinary team and direct supervisor as needed.
11. Address ongoing clinical and/or systemic needs - with attention to potential risk factors.
12. Provide weekly updates assessing client's clinical, safety, and stabilization needs

Required education and experience
Bachelor’s degree required
Minimum of 2 years of experience working in the Addiction Recovery field, including internship placement hours.
