Riverbrook Healthcare is hiring a MDS COORDINATOR (RN or LPN) to work both short term rehabilitation and long-term care.

Position Summary: Under the supervision of the Director of Nursing, the MDS Coordinator will coordinate the Resident Assessment Instrument (RAI) process to ensure compliance with accuracy attestation and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA and other payor program requirements. The MDS Coordinator ensures appropriate documentation to report and support services provided and assessment accuracy as well as communicates effectively with other members of the interdisciplinary team. This position requires the ability to work well with residents, families, and staff.
Requirements:
The Licensed Practical Nurse or Registered Nurse must be a graduate of an approved school for Practical or Vocational Nursing and must be currently licensed by the State Board of Nursing.
MDS and RAI experience is preferred in geriatric or rehabilitative Nursing.
