At RiverWoods Exeter, we are seeking a skilled Nursing Professional to join our team on a per diem basis. This opportunity allows you to provide direct resident care in a team-focused environment during daytime shifts.
Key Responsibilities:
- Perform professional duties in accordance with facility policies and standards of professional practice, State of New Hampshire Nurse Practice Act, OBRA, and HIPAA.
- Collaborate with interdisciplinary team members to formulate individualized resident care plans, updating them as necessary and participating in Resident Care Plan Meetings.
- Institute care protocols, including falls prevention, safety, behavior management, weight management, skin integrity, bowel/bladder management, infection control guidelines, and pain assessment and management.
- Assess and document changes in residents' physical or mental status, reporting findings to the primary care physician and treatment team. Provide complete clinical data for informed decision-making.
- Supervise direct care through observation and evaluation of results.
- Foster positive working relationships with team members, residents, and family members to facilitate open communication.
- Administer medications and treatments according to professional guidelines and nursing department policies, managing narcotics inventory and storage securely.
