Overview
The MDS Coordinator plays a crucial role in the healthcare setting, ensuring that patients receive appropriate care through effective management of the Minimum Data Set (MDS) process. Duties
- Conduct comprehensive assessments of patients using the MDS framework to determine their care needs.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Ensure accurate documentation and coding of patient data in compliance with ICD-10 standards.
- Perform utilization reviews to assess the appropriateness of patient admissions and ongoing care.
- Maintain knowledge of managed care practices and regulations affecting patient care.
- Utilize electronic health record systems, such as Epic, to document assessments and coordinate care.
- Provide education and support to nursing staff regarding MDS processes and documentation requirements.
- Stay updated on changes in regulations related to MDS assessments and reporting.
Skills
- Strong understanding of anatomy and physiology as it relates to patient assessments.
- Experience working in a Level II Trauma Center or Pediatric Intensive Care Unit (PICU) is preferred.
- Proficiency in utilization review processes and managed care principles.
- Familiarity with medical office operations and nursing home environments.
- Excellent communication skills for effective collaboration with healthcare professionals and families.
- Detail-oriented with strong organizational skills for managing multiple assessments and documentation tasks.
