What are the responsibilities and job description for the Public Health Nurse position at Rock County?
Public Health Nurses in the Communicable Disease, Chronic Disease and Injury Prevention, and Maternal, Child and Family Health units focus on population health, with the goal of promoting health, preventing disease and disability, and achieving more equitable health outcomes. Public health nursing practice considers all levels of prevention with a preference for primary prevention. Most services are provided at the community and system level, conducting policy, system, and environmental interventions to the population or sub-populations to assure the health department's role as Chief Health Strategist. At the Rock County Public Health (RCPH), Public Health Nurses work in collaboration with other public health staff and community partners to identify, implement, and evaluate strategies to improve population health. This position is focused on Chronic Disease & Injury Prevention, with a primary focus on facilitating Rock County’s Overdose Fatality Review and developing overdose prevention strategies.

- Obtain and maintain data that provide information on the community's health (e.g. provider immunization rates; hospital discharge data; public health hazards, risks, and exposure data; community-specific health indicators, etc.).

- Maintain statistical expertise and capacity to help monitor community health status.

- Review the effectiveness of public health interventions provided by other practitioners and agencies for prevention, containment, and/or remediation of problems affective the public's health, and provide expertise to those interventions that need improvement.
- Engage the community to identify gaps in culturally-competent, appropriate, and equitable personal health services, including preventative and health promotion services, and develop strategies to close the gaps.
- Support and implement strategies to increase access to care.
- Link individuals to available, accessible personal healthcare providers.
- Exchange information and data with individuals, community groups, other agencies, and the general public about physical, behavioral, environmental, social, economic, and other issues affecting the public's health.

- Maintains confidentiality of protected health information (PHI) of individuals, identifiable data elements and small data sets, including but not limited to only accessing information related to assigned work, utilizing locked filing cabinets, electronic password protection, and encryption when creating, handling, and storing documents that contain PHI.

- Current cardiopulmonary resuscitation (CPR) for healthcare providers certification, or ability to receive within 30 days of hire.
- Completion of required National Incident Management System (NIMS)/Incident Command System (ICS) trainings within 6 months of hire.
- Master's degree in Public Health (MPH)

- Public health related professional experience, public health training, or population level health initiative experience.

Working Conditions

- Ability to travel within and outside of the county, including over-night stays, to attend trainings and conferences.
