The incumbent of this position works with the interdisciplinary team and administration of the Rock Haven Nursing Home to promote and protect residents' rights and the psychosocial well being of each resident.  By coordinating medically related social services at Rock Haven, the incumbent assists each resident to attain or maintain his/her highest practicable physical, mental and psychosocial well-being. The incumbent assigned to this position will take responsibility for the care plan development for  specialized services offered within the facility.

50% - Participates with the I-team in the overall planning and implementation of the resident's plan of care.  

-Completes the psychosocial assessment and social history upon admission, quarterly and as needed. Completes documentation in a timely manner.

-Observes, reports and documents pertinent changes in the resident's physical, emotional/behavioral or social status.

-Evaluates and documents the resident's response to interventions. Develops discharge planning to reflect the resident's potential to return to the community.

-Participates in Interdisciplinary team meetings, Medicare meeting and other committees/meetings as assigned.

-With the I-team, assesses the need and nature of counseling or psychiatric services for residents including community mental health programs.


35% - Provides individual, group and family services related to admission and discharge planning and resident adjustment to the facility.  -Interacts with residents, families, guardians and representatives of other agencies on an ongoing basis.

-Develops a knowledge base of community resources that are available to the long-term care resident as well as those residents with discharge potential.

-Is pro-active in supporting and utilizing community resources.

-Provides time-limited counseling services to the residents as deemed appropriate.

-Develops the specialized service program/ care plans and reviews  for those clients who require these services.

10 % Prevents abuse/neglect and violation of resident rights by assisting in the preparation and presentation of annual in-services related to these issues as well as dementia, chronic mental illness and other psychosocial issues as assigned.

-Promptly reports any indication of resident abuse or neglect (per facility policy).

-Conducts quality assurance activities related to quality indicators and measures, psychoactive medication use and related topics as assigned.

-Assists the team to identify and correct areas of concern for the residents.


5% - Other duties as assigned.

   

-Knowledge of social work principles, techniques and practices and their application to the long-term care setting.
-Considerable knowledge of a wide range of behavior and psychosocial problems and their diagnosis and treatment.
-Considerable knowledge of family and group dynamics.
-Skill in establishing rapport with a resident and in applying techniques or assessing psychosocial, behavioral or psychological aspects of resident's problems.
-Working knowledge and awareness of state and federal requirements related to the provision of social services skilled nursing facilities.
-Ability to communicate effectively orally and in writing.
-Demonstrates responsibility and accountability for professional practice and for continued professional growth and self-evaluation.

 -Bachelors required with a degree in social work or related field. 
-Professional experience with a geriatric population.
-Certified Social Worker preferred.-Ability to sit, stand, bend, and stoop for periods of time
-Ability to lift up to 50 pounds occasionally.
-Ability to work in a stressful environment.
-Ability to respond to emergency/crisis situations.
-Ability to push/pull a medication cart.-
-Exposure to noise.
-Exposure to blood and/or body fluids.
-Uses personal protective equipment as needed but not limited to: gloves, gowns, goggles and protective shield.-
Ability to enter accurate information into charts and reports.
-May be subject to potentially hostile and emotionally upset residents, family members, personnel, visitors,   etc.
-May be subject to falls, burns, odors, etc., throughout the work day.
-May be exposed to infectious waste, diseases, blood, body fluids, conditions, etc.-
-Work is inside in a skilled nursing home facility.
-CPR/AED certification required
