Job Description :
Join our dedicated team at Rockaway Home Care, a licensed home care agency serving New York’s downstate area. We are seeking a compassionate and skilled Registered Nurse (RN) to conduct assessments and supervise home health aides in patients' homes. This role is crucial for ensuring our patients receive the highest quality care.
Responsibilities :
Conduct comprehensive assessments and aide supervisions in patients' homes.
Perform thorough head-to-toe patient assessments, including medications and diagnoses, and document findings accurately.
Supervise and support home health aides / personal care aides in all duties.
Develop and implement personalized Plans of Care (POC) for aides to follow and provide orientation to the POC.
Position Requirements :
Current and valid NYS RN License.
Ability to travel to patient cases within the 5 boroughs / Nassau / Suffolk.
Must carry Medical Professional Liability coverage.
Intermediate proficiency in using computerized systems such as Gmail, Microsoft Word, Excel, and the DOH UAS health care system.
Preferred Qualifications :
Bilingual in Spanish / English.
Experience with Medicaid, Medicare, and Managed Care.
Experience in home and community-based or long-term care services.
Proficiency in using the UAS-NY application.
Experience working with vulnerable populations and a passion for patient care.
Excellent written communication and documentation skills.
