DESCRIPTION:
The Case Manager will serve as a participant advocate and assist with all admission functions. This individual will be responsible for the development, implementation and evaluation of social service care plans for each assigned participant, coordinating services with other agencies/caregivers and continuously evaluating all service and care plans to see that needed services are received. Experience in Dementia setting is highly preferred.
JOB QUALIFICATIONS:
Bachelor's degree in social work or human services from an accredited college and at least one year of related work experience Must possess sensitivity to the needs of disabled and low-income older adults. Must be able to work cooperatively as a multi-disciplinary team member.
SPECIFIC RESPONSIBILITIES:
Social Services:
- Coordinate services with other agencies; acts as primary case manager if necessary.
- Provide both individual and group meetings with participants and their caregivers.
- Participate in family meetings and case conferences.
- Develop and update social service plans (admission and progress notes, discharge plans as required by Department of Medical Assistance.
- Keep current knowledge of community resources and monitor care plans to see that necessary and available services are received by participants.
- Assist in crisis intervention as necessary.
- Provide intervention in participant behavioral issues.
- Make referral to community services as needed.
- Assist applicants and participants in application process for benefits only when no other advocate is involved.
- Assist with all admission functions, including: Receiving referrals and informational phone calls, coordinating application process on prospective participants, gathering pertinent information from other providers (i.e. physicians, home care agency, social workers) providing tours and scheduling guest and admission days. Work closely with nursing to determine applicant eligibility and in conjunction with nurse make recommendation to Day Program team regarding appropriateness for admission.
- Receive informational calls, and if applicant is not eligible for admission to the Adult Day Programs, makes appropriate referrals.
- Participate in Weekly Clinical meetings.
