Case Manager
Compensation: $17-$20 per hour
Reports to Executive Director
Position Summary:
The Case Manager at Room at the Inn is a vital member of our team, responsible for providing comprehensive case management services to individuals experiencing homelessness. The Case Manager will work closely with clients to assess their needs, develop individualized service plans, and connect them with resources and support services to help them achieve stability and independence. The Case Manager plays a key role in assisting clients in their journey towards permanent housing and self-sufficiency.
**The current opening is for a case manager to work intensively with a small case load. Must be able to be at the shelter 3-4 evenings a week to meet with clients and prepare treatment plans, then work days to help clients achieve agreed upon goals. This position will focus on helping elderly individuals gain entry to nursing homes and work with those who have long histories of homelessness without progress toward housing.**
Responsibilities:
Client Assessment:
- Conduct comprehensive assessments of clients to identify their strengths, needs, and barriers to stable housing.
- Utilize assessment tools to gather information on housing history, employment, medical conditions, mental health, substance use, and other relevant factors.
Individualized Service Planning:
- Develop individualized service plans in collaboration with clients, setting achievable goals and objectives.
- Regularly review and update service plans to ensure they remain responsive to clients' changing needs and circumstances.
Housing Support:
- Assist clients in identifying and applying for appropriate housing options, including emergency shelters, transitional housing, and permanent supportive housing.
- Coordinate with housing providers and landlords to secure stable housing options for clients.
Resource Referrals:
- Connect clients with a wide range of resources, including mental health services, substance abuse treatment, medical care, employment assistance, and educational programs.
- Collaborate with community partners and agencies to facilitate access to necessary services.
Crisis Intervention:
- Provide crisis intervention and support to clients in times of immediate need or emergency situations.
- Develop safety plans for clients facing potential crisis situations.
Advocacy and Support:
- Advocate on behalf of clients to access housing, employment, and other necessary services.
- Offer emotional support and encouragement to clients during their journey towards stability.
Progress Monitoring and Documentation:
- Maintain accurate and up-to-date case files for each client, documenting all interactions, progress, and outcomes.
- Regularly track clients' progress towards achieving their service plan goals.
Collaboration and Teamwork:
- Work collaboratively with other case managers, shelter staff, and external service providers to ensure coordinated and integrated services for clients.
- Participate in case management team meetings, staff meetings, and training sessions as required.
Compliance and Reporting:
- Ensure compliance with all relevant regulations, funding requirements, and reporting obligations.
- Complete and submit necessary documentation and reports accurately and on time.
Qualifications:
- Bachelor's degree in social work, psychology, human services, or a related field.
- Previous experience in case management, social work, or related field.
- Knowledge of homelessness and poverty-related issues, mental health, and substance abuse.
- Strong communication and interpersonal skills, with the ability to establish rapport and build trusting relationships with clients.
- Proficient in conducting assessments and developing individualized service plans.
- Familiarity with community resources and services available to individuals experiencing homelessness.
- Ability to work independently and as part of a team, demonstrating flexibility and adaptability.
- Strong organizational skills and attention to detail.
- Compassionate, non-judgmental, and culturally sensitive approach to working with diverse populations.
- Working knowledge of evidence-based practices such as trauma informed care, strengths-based approach, and motivational interviewing.
- Flexibility to work mixed shifts including evenings and days as needed.
- Good driving record.
