We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will be responsible for providing support and guidance to clients facing various challenges, including transitioning from incarceration back into the community, behavioral health issues, addiction, and child welfare concerns. This role involves developing individualized plans, coordinating services, and advocating for clients to ensure they receive the necessary resources for their well-being.
Duties
- Conduct comprehensive assessments to identify client needs and develop personalized service plans.
- Utilize motivational interviewing techniques to engage clients and encourage positive behavior change.
- Provide mentoring and support to clients in navigating workforce development opportunities.
- Supervise and coordinate activities with other service providers to ensure holistic care for clients.
- Address issues related to child welfare by collaborating with families and relevant agencies.
- Offer crisis management support during critical situations, ensuring client safety and well-being.
- Facilitate access to addiction counseling services as needed, supporting clients in their recovery journey.
- Maintain accurate documentation of client interactions, progress, and outcomes in compliance with regulatory standards.
- Assist clients with childcare resources and Section 8 housing applications when applicable.
Qualifications
- Bachelor’s degree in social work, psychology, or a related field is preferred; relevant experience may be considered in lieu of a degree.
- Proven experience in case management or a similar role is highly desirable.
- Strong understanding of behavioral health principles and child welfare regulations.
- Excellent interpersonal skills with the ability to build rapport with diverse populations.
- Proficient in crisis intervention techniques and conflict resolution strategies.
- Familiarity with workforce development programs and resources is a plus.
- Ability to work independently as well as collaboratively within a team environment.
- Strong organizational skills with attention to detail in documentation and reporting processes.
