Position Summary: The Clinical Programs Manager develops and oversees clinical program activities, supervises clinical programs staff, and works collaboratively with clinical services and other teams at Roots. The Clinical Program Manager ensures all clinical program activities are conducted in a timely and effective manner and meet program goals, expected deliverables, and compliance requirements. The Clinical Programs Manager, with support from clinical directors as well as the clinical data and administrative team, conducts timely follow‐up activities, manages clinical program activities, and ensures timely and accurate data entry and submission to management for review prior to external transmission, when applicable.
Duties and Responsibilities
- Manage the development, implementation, expansion and ongoing refinement of clinical programs (currently including but not necessarily limited to: Diabetes, Asthma, HTN, Substance Use, HIV, Hep C, and Covid/flu).
- Provide direct supervision and support to clinical program coordinators and specialists
- Support program coordinators in community outreach activities and events while maintaining related relationships with clinical, community-based and neighborhood organizations.
- Ensure timely and high-quality completion of all program deliverables.
- Manage the ongoing process of identifying members who are most at risk of poor health outcomes and in need of care management, group engagements, panel management and other clinical program offerings.
- Identify opportunities for outreach and “in-reach” for clinical programs.
- Ensure adequate utilization of clinical program resources.
- Ensure timely and accurate reporting as required by contract guidelines, with support from clinical program directors and clinical program administrators
- Support the development of funding proposals for clinical programs.
- Communicate with providers, members, and community resources as necessary to support the planning, implementation, and evaluation of clinical programs.
- Establish and/or maintain working and collaborative relationships in the community, on behalf of and in accordance with formal understandings of Roots Community Health.
- Develop relationships with external entities for referrals to and from Roots.
- Work closely with Clinical Directors to ensure improved clinical outcomes are being achieved through clinical programs.
- Support the development, management, and tracking of budgets and spending.
- Develop reports and presentations to convey program progress and compliance with established standards, contracted goals, and objectives.
- Make and recommend personnel decisions including but not limited to hiring, promotion, transfer and assignment of staff, and performance improvement plans.
- Work with clinical staff and leadership to help identify merging needs and gaps for our members and the broader community.
- Provide direct clinical medical services in the area of licensure (if applicable) in accordance with the highest applicable standards of medical and professional practice and in full accordance with health center protocols and policies.
- Implement a trauma‐informed and compassionate care approach to program delivery from both a patient care and staff wellness perspective.
- Ensure staff compliance with HIPAA and policies and procedures for client consent and information sharing in program delivery.
- Participate in community, agency and department committees and meetings.
- Complete all required documentation and reports in a timely fashion.
- Participate in staff meetings, agency meetings and staff trainings.
- Carry‐out all assigned responsibilities in a professional manner.
Qualifications
- Masters in Public Health with minimum 2 years’ experience managing or coordinating clinical programs, OR Registered Nurse with graduation from an accredited school of Nursing and possession and maintenance of a valid license as a Registered Nurse issued by the State of California Board of Registered Nursing and minimum 1 years’ experience managing or coordinating clinical programs
- Minimum 2 years of experience working in a non‐profit organization, or a community clinic preferred.
- Strong managerial and team-building skills
- Possess and maintain a valid California driver license.
- Comfortable working with marginalized populations, particularly low-income communities of African descent. Comfortable working with individuals who are homeless, formerly incarcerated, and/or dealing with substance use, HIV or other chronic conditions.
- Availability to work occasional weekends as needed for community events.
- Experience analyzing and compiling reports.
- Experience working across disciplines including Community Health.
- Excellent written and verbal communication skills.
- Knowledge of patient and staff education methods.
- Knowledge of governing federal, state and local laws, rules, regulations; and policies related to nursing, Medi‐ Cal, patient privacy and consent in a shared care environment.
- Ability to plan for, organize and manage projects in a dynamic environment.
- Proficient in computer use, including MS Office and G-Suite.
- Ability to establish and maintain effective working relationships.
