What are the responsibilities and job description for the QIDP/Case Manager position at Rose-Mary?
Rose-Mary is searching for a dynamic team member to fill the role of Qualified Intellectual Disability Professional (QIDP). This person will oversee the Individual Habilitation Plan for individuals with intellectual and developmental disabilities residing in community group homes. We are looking for a candidate that is a good communicator, well organized and has experience in this field.
Minimum Qualifications:
- One year experience in ID/DD or closely related field required, two or more years preferred.
- Demonstrated ability for self-direction in work
- Excellent verbal and written communication
- Valid Ohio Driver’s License
- Bachelor’s degree required
- Proficiency of Microsoft Office Products – Word, Excel, PowerPoint, Outlook
Primary Responsibilities:
- Quality assurance & training - Coordinates and trains the Direct Support Professionals in the implementation of each Individual habilitation plan. Monitors and evaluates the provision of services, and provides training on individual plans and the general support of individuals to encourage continuous quality improvement.
- IHP development & problem solving - Evaluates each individual’s needs, strength, preferences, and abilities, and assures that plans stay updated with what is important to and for each individual. Communicates with team members to identify and problem-solve new issues that arrive for individuals.
- Team facilitation - Acts as a team facilitator for Interdisciplinary Team Meetings, which include initial admission meetings, annual and quarterly reviews, and special habilitation meetings.
- Guardian communication - Engages guardians as active members of individuals’ teams. Proactively communicates to build trusting relationships with guardians. Obtains consent from guardians when needed, and problem solves guardian concerns when they arise.
- Professional relationships - Develops positive professional relationships with DSPs, supervisors, managers, nurses, administrators, and community partners. Invests in relationships and engages DSPs as critical members of interdisciplinary teams.
- Incident reporting - Maintains effective communication and records of UIRs and MUIs according to proscribed timelines.
- Documentation & record keeping - Maintains individuals’ documentation and electronic files, keeps files up to date per agency policy and procedure. Documents events and discussions to anticipate future oversight and team needs.
- Regulatory knowledge & education - Maintains familiarity with current Licensure and Certification standards as they apply to ICFs. Attends periodic training opportunities for professional growth.
- Person-centered behavior support - Promotes the use of Good Life & trauma responsive approaches to behavior support and mental wellness for individuals. Communicates individual needs and progress with staff, psychiatrists, and other consultants & team members.
