Rothman Orthopaedic Specialty Hospital is a single-specialty surgical hospital in Bensalem, Pennsylvania. Our hospital was developed by Rothman physicians to provide their patients high quality and patient-focused care. Our facility features 6 fully-equipped operating rooms and 24 in-patient rooms. We are equipped for joint replacements, orthopedic surgery, pain management and spine surgery, as well as sports medicine, foot and ankle, shoulder and elbow, and hand and wrist procedures.

JOB SUMMARY:

The Pharmacist is a member of the Patient Safety Committee and is responsible for the control and delivery of drugs and shall have the responsibility for the pharmaceutical functions of the Nursing Stations. The Pharmacist is responsible for developing, supervising and coordinating all pharmaceutical activities.

QUALIFICATIONS :

• Bachelor of Science Degree in Pharmacy or PharmD degree from an ACPE-accredited school of pharmacy, or equivalent
• Current Pharmacist PA license
• BLS Required

RESPONSIBILITIES:

1. Monitoring and auditing all narcotic activity throughout the facility, including controlled substance ordering and inventory management. Activity includes monitoring narcotic medications, creating par levels, filling and refilling automated dispensing machines used by all areas of the hospital. Maintain the Biennial inventory of controlled substances per DEA regulations.
2. Inventory control and formulary management considering possible deletions or additions, considering options for dealing with any drug shortages and any therapeutic substitutions.
3. Monitoring the filling and delivery of medication requisitions.
4. Medication reconciliation of patient home medications brought to the hospital for use while they are admitted. Identify the medications and verify that they were entered correctly, making any necessary changes, and talking to the patient if clarification is needed.
5. Provide staff education on proper Medication reconciliation process, Code cart medications, Malignant hyperthermia medication prep and PCN allergy documentation and workflow design.
6. Preparation and labeling of medications using the Pharmacy Glove box. Includes documenting and tracking all medications prepared using the Glove Box in Compounding Record and Master Formulation Record.
7. Ensure Glove box is cleaned, disinfected and maintained by pharmacy staff daily per state and federal regulations.
8. Ensure environmental monitoring is performed for the Glove box which includes, fingertip sampling, surface growth sampling tests, viable and non-viable air sampling, smoke pattern test, temperature and humidity control.
9. Ensures Quality Assurance in hospital pharmacy through policies and procedure, personnel training/education and evaluation, storage and handling, facilities and equipment, hand hygiene, garb, aseptic technique and product preparation, process validation, expiration dating, labeling, documentation, end-product evaluation and environmental monitoring,
10. Manages and reviews annually staff competency and certifications to compound.
11. Manage order sets and medication dictionary to ensure safe, accurate and efficient medication orders for CPOE and pharmacy modules. Update orders for physicians, NP and PA's as needed.
12. Monthly inspections of all units looking at proper drug storage, expirations, proper monitoring and documentation of narcotic records, drug safety, and overall compliance.
13. Monthly reports of the Pharmacist inspections are maintained.
14. Provide drug information to staff whenever requested and, more formally, during any in-services.
15. Provide information and Pharmacy related orientation for non-pharmacy personnel.
16. Investigate any medication errors and adverse drug events, and respond in ASC WebQI.
17. Maintain and update all policies and procedures through Policy Stat and through the P&T Committee.
18. Maintain the Pharmacy Dashboard which tracks drug use of select medications, Pharmacy PI results, Pharmacist interventions, Pharmacy cost savings, Medication errors and adverse drug events.
