As the LCSW, you will deliver social work services and programs to patients and caregivers utilizing social work theory, principles and methods to provide strength based assessment, evaluation, formulation and implementation of a treatment plan to address the psychosocial aspects of illness, it’s impact on the patient and caregiving system; provides education and referral to community resources, may provide clinical services and psychotherapy with the goal of resolving psychosocial dysfunction and eliminating barriers to effective disease management. You will also serves as a Patient Navigator for patients at Roswell Park and the Care Network to ensure that continuous quality care is received by all patients; assists with the connection of patients from community and clinical referrals for assessment and treatment; assists patients in accessing needed resources and services to allow them to move through the spectrum of cancer care and into long-term survivorship; coordinates with internal services and outreach programs with local and regional health centers, transportation services, to facilitate timely cancer care, and encouragement of adequate patient adherence; serves as a resource for hospitals, screening facilities, county health units, state health department, patients, physicians, lay public, volunteers statewide, and graduate students into resources and information regarding Roswell Park.
Responsibilities:
- Provides diagnostic assessment of mental, behavioral, emotional, addictive and developmental disorders/disabilities of individuals, families and groups and the aspects of illness, injury and impairment within a psychosocial framework and within the scope of licensure; documents diagnoses and assessment in the medical record.
- Develops and implements assessment-based treatment plans and an integrated plan of prioritized clinical interventions based upon the treatment plan to address impairments, enhance functioning and modify maladaptive behavior of the patient and caregiving system.
- Provides distress management, lethality assessment, crisis oriented and brief short-term psychotherapy to individuals, groups and families and serves as a liaison between patients and families in distress and staff in times of extreme emotional distress.
- Provides clinical counseling to individuals, families and groups regarding the psychosocial aspects of illness, disability, grief and bereavement designed to ameliorate emotional and social barriers to disease management.
- Acts as a member of the multidisciplinary team regarding psychosocial issues and concerns; participates in formal and informal family, psychosocial and multidisciplinary conferences regarding these issues; acts as a liaison between patient, family, and hospital staff during times of crisis and extreme emotional distress.
- Documents assessment, goals, plans, problems and interventions in the medical record.
- Prepares written documentation as required by the profession and the department; maintains, distributes, analyzes and projects information for required records, reports and statistics as directed.
- Works directly with patients to assess personal, physical, cultural, and other needs that impact treatment adherence and patient satisfaction.
- Recognizes what needs can be matched with what resources and delegates resolution to appropriate staff.
- Enhances adherence to patient follow-up by (1) identifying barriers, (2) streamlining appointments and paperwork, (3) assisting in filling out forms, (4) keeping communication open with providers, caregivers and administrators within and outside Roswell Park, (5) providing assistance in getting culturally appropriate, supportive care when and where possible, (6) linking patients, caregivers and families with follow-up services, and (7) keeping in contact with patients while they are receiving care by calling them prior to appointments and tracking their progress and evolving needs.
- Works with the clinical staff and administration to assure patient-provider communication is optimized and that physicians and nurses are well informed about patient issues that impact treatment.
- New York State licensure as a Licensed Clinical Social Worker (LCSW).
- Master’s degree in Social Work from a Council on Social Work Education (CSWE) accredited facility
- Minimally one (1) year of full-time experience in Medical Social Work.
- Bereavement counseling, and/or experience in a comprehensive cancer center required.
