We are searching for a Services Intake Case Manager to facilitate the initial intake process into RS EDEN Support Services programs, including establishment of eligibility and coordination of move into permanent supportive housing. This position plays an integral role in supporting single adults, youth, and families transitioning from homelessness or housing instability into housing, while providing compassionate and efficient wraparound services
RESPONSIBILITIES
- Screens all potential clients for eligibility for Housing Stabilization Services and supportive housing programs, including collecting accurate and consistent eligibility documentation for a variety of funding streams for each incoming client.
- Works collaboratively with multiple team members, including but not limited to RS EDEN Property Management, RS EDEN Support Services Staff, Alliance Apartments Support Services staff, and Aeon Property Management staff.
- Represents RS EDEN housing to the community at large via positive relationships with collateral agencies and community providers.
- Maintain electronic health records in accordance with the expectations for timely and thorough billing for services rendered.
- Transportation of clients in personal vehicle within the Twin Cities metro area.
Pay Range: $43,000-$47,000/annually ($20.67-$22.60/hour)
REQUIRED QUALIFICATIONS
- Current and valid driver's license, and a clean driving record
AND
- High school diploma/GED and 6 years of case management, supportive housing, mental health services, substance use treatment, or related field
OR
- Associate’s degree in Human Services or related field and 4 years of case management, supportive housing, mental health services, substance use treatment, or related field
OR
- Bachelor’s degree in Human Services or related field and 2 years of case management, supportive housing, mental health services, substance use treatment, or related field
PREFERRED QUALIFICATIONS
- 2 years of experience working in homeless response or supportive housing
- Excellent verbal and written communication skills
- Possess intermediate computer skills which includes a moderate understanding of operating systems, word processing, spreadsheets, and databases
- Ability to relate to people from diverse backgrounds with multiple individual and systemic barriers to success
