Hospice Registered Nurse Case Manager
We are seeking a compassionate and skilled Hospice Registered Nurse to deliver personalized patient care in Northern Fairfield County.
Key Responsibilities:
- Regularly assess and reassess nursing needs of Hospice patients, implementing care for comfort and symptom palliation
- Provide direct client care and collaborate with case managers to implement care plans
- Perform admission procedures using electronic health records, developing plans consistent with physician orders and patient wishes
- Coordinate plan implementation for patients in SNF, ALF, hospital contract beds, or personal residences
- Offer dietary counseling and provide Hospice nursing services, treatments, and preventive procedures
- Initiate nursing procedures ensuring patient safety and reporting unexpected changes in physical or emotional condition
- Teach, supervise, and counsel patients, family members, and facility staff on providing care
Requirements:
- Bachelor's degree in Nursing; BSN preferred
- Current licensure as a Registered Nurse in Connecticut
- A minimum of two years' experience in medical/surgical, oncology, Critical Care, ED, or labor and delivery nursing; Home Health Care and/or Hospice experience preferred
- Ability to work independently, make accurate judgments, and drive in various weather conditions
