What are the responsibilities and job description for the Case Manager position at S.H.A.R.E. House, Inc.?
Case Manager needed for a Domestic Violence Intervention Center. The successful candidate must have experience in developing case plans, case coordination, resource development, and follow up services. This position requires transporting clients to appropriate appointments. Knowledge of life skills, activity development and planning. The candidate must be able to document activities/services provided. The candidate must be able to serve as a group leader to include parenting. Skills are needed in effective listening, communication and documentation. This position will require some flexibility to include some evenings and week end work. Prefer experience in the areas of Domestic Violence and Social Services.
Responsibilities include but not limited to:
Develop strong relationships with The Department of Family and Children Services
Serve as a group leader for Domestic violence support groups, Parenting and Life SKills
Developing Case Plans with victims
Developing Safety Plans and updating as required.
Perform home visits to victim's residing in the agency housing programs.
Provide life skills training.
Enter data into the state's case management system
Document all services in the agency case management system.
Effectively work as a TEAM member.
Qualifications
Minimum of a Bachelor's Degree in Social Services and or related field.
A minimum of two (2) years of Social Services experience.
Computer Skills
Good writing and communication skills
Excellent Listener
Excellent time management
Excellent documentation skills
