What are the responsibilities and job description for the LPN Team Coordinator position at SAAD ENTERPRISES?
Saad Hospice is hiring Team Coordinators to help facilitate our daily functions of our Field Hospice Services. This job does requires you to work every other weekend, have great phone etiquette, and be a team player.
Coordinate with the Clinical Director to ensure proper direction of people and resources for the Hospice Department.- Responsible for reviewing communication log daily and following up with resolutions added into the log.
- Assist the Quality Assurance Leaders and the Clinical Director in measuring quality of work in documentation and in person.
- Act as a resource for patient education and information, instructing patients/caregivers concerning treatments, test results and other information as needed.
- In coordination with the Clinical Director, they determine the urgency of seeing the patient based on brief assessment and on familiarity with a patient's condition.
- Able to display professionalism when triaging emergent situations.
- Coordinates with the holistic care team to facilitate efficiency of services.
- Understand the needs of the organization and support the mission, values, and management of patient care services.
- Administers hospice care to patients on an intermittent basis in the home or IPU.
