OVERVIEW: Under the supervision of the Clinical Program Manager, the Mental Health Team Specialist provides the team and client/families with direct support in coordinating communication and collaborating with adjunct care providers. The Mental Health Team Specialist also provides assessment, parenting support and linkage to resources and referrals to clients and their families to maximize the implantation of plans and facilitate the integration, coordination and implementation of services.
DUTIES:
- Coordinate, schedule, and facilitate child and family team meetings (CFT).
- Coordinate with teachers, social workers and other adjunct providers and treatment teams.
- Provide collateral and parenting support.
- Monitor and ensure linkages and outsourcing of resources with community and agency service providers.
- Provide crisis case management, including collaboration with discharging hospital, obtaining appropriate hospital discharge records and coordination of discharge planning.
- Administer and score assessments, as assigned.
- Complete Child Health Questionnaire as required.
- Complete intake assessments for new referrals and support direct requests for services.
- Provide an array of direct service to clients/families, as needed.
- Enter assigned client/family data entry in Electronic Health Record.
- Perform Utilization Reviews, as assigned.
- Write daily progress notes for each service (billable and non-billable) delivered within the timelines noted in SCH documentation policy.
- Participate in staff meetings, supervision, case reviews, curriculum and other meetings as required.
- Participate in Sacramento Children’s Home activities and training as required.
- Establish and maintain positive and effective working relationships with internal staff and external resources.
- Adhere to policies, systems and standards of the Sacramento Children’s Home and all applicable programs, agencies and contracts.
- Carry out mandated reporter duties as required by law.
- Perform other duties as assigned.
LOCATION: 2750 Sutterville Rd. Sacramento, CA 95820
SALARY: $$25.91 – 29.18, per hour, to start
Qualifications
Education and Experience
At least four years of full-time experience working in the mental health field and a Bachelor’s degree is required. An associate’s degree is allowable with at least 6 years of full-time experience working in the mental health field. A Master’s degree is allowable with at least 2 years of full-time experience working in the mental health field.
Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.
