Sacred Circle Healthcare is seeking two full-time, Medical Behavioral Health Therapists, in Salt Lake City, Utah.
Job Summary
General Summary of Duties : The Medical Behavioral Health Therapist (MBHT) assists in with providing Behavioral Health support to the medical teams through general team assistance and ensuring high quality services are provided in accordance with all compliance regulations and requirements. The MBHT creates a supportive, client centered environment that is led by SCHC’s mission, values and strategic goals. The MBHT will carry an average clinical caseload of 28 scheduled hours per week. The position of MBHT is a full-time position, based on a 40-hour work week comprised of either 4 or 5 working days each week

The successful candidate :
- Conducts intake assessments to gather, analyze, and evaluate psycho-social data according to policy and procedures and in compliance with all state and licensing regulations, I.H.S. standards, Center for Medicare and Medicaid (CMS) requirements, and AAAHC standards regarding service delivery and documentation.
- Provides mental health counseling to children, adults, couples, and groups. May be counseling at the Reservation in Ibapah, UT 1-2 days a month (travel paid for) according to policy and procedures and in compliance with all state and licensing regulations, I.H.S. standards, Center for Medicare and Medicaid (CMS) requirements, and AAAHC standards regarding service delivery and documentation.
- Assists medically based providers with client evaluations concurrent with a clients medical appointment to establish a referral for individual / group therapy.
- Coordinates care of patients with primary care providers regarding psychotropic medication needs.
- Provides behavioral health interventions related to a variety of needs such as smoking cessation, medication compliance, weight control, etc.
- Creates case conceptualization based on appropriate diagnosis and best treatment standards, presents the findings at Clinical Case Staff meetings, documents peer feedback and includes feedback into case conceptualization.
- Completes Participates in collaborative treatment plans planning with patients, incorporates treatment goals and objectives into sessions, documentation and updates the plans them every 6 months or as needed.
- Completes notes timely documentation of all clinical contacts including crisis, intake / evaluations, diagnoses, tx planning, referrals and outcome measures and and reports in a timely manner (within 72 hours of visit for notes).
- Participates in creating a positive, supportive team environment.
- Evaluates outcome data and the therapeutic relationship to ensure services are delivered in an effective manner
- Maintains strict confidentiality
- Responsible for maintaining licensure.
- Assists with weekly clinical case staff meetings which includes : case presentations, discussion of treatment plans, a forum to process clinical challenges with peer feedback, as well as any training and dialogue on relevant clinical best practices.
- Reviews reporting / documentation to ensure compliance with all state licensing regulations, I.H.S. standards, Center for Medicare and Medicaid (CMS)requirements, and AAAHC standards regarding service delivery and documentation.
- Maintains compliance of SCHC Policy and Procedures.
- Participates in creating a positive, supportive team environment using SCHC’s mission, values and oversees the day-to-day BH operations
- When requested partners with HR for clinic training consultation, clinic lunch and learns as well as community education on mental health topics as needed.
- Participates in crisis and de-escalation services for the clinic patients and staff as needed.
- When requested partners with the Community Connections Outreach team to assist in community events and support.
- Meets regularly with the Manager for guidance, and to ensure the clinic vision is implemented.
- Attends requested and required meetings.
- Reads, understands, and adheres to all Sacred Circle Health Care Policies and Procedures.
- Practices and adheres to the Code of Conduct and Mission and Values Statements.

Minimum Requirements
EDUCATION :
Master's degree in Psychology, Social Work or related field and has a license
EXPERIENCE :
1-2 years in an organization / clinic that services both Medical and Behavioral Health.
1-2 years providing Behavioral Health therapy with a focus on Pain Management.
CERTIFICATION / LICENSE :
Current Utah Clinical License in good standing.
Other :
High level of approachability and communication skills.
Displays leadership characteristics including the ability to consistently maintain company policy and procedures as demonstrated by personal workstyle
