Position Overview:
We are currently seeking a skilled and compassionate Physical Therapist to join our team of healthcare professionals. The Physical Therapist will play a vital role in helping our clients achieve optimal physical functioning, independence, and overall well-being. Responsibilities:
- Conduct comprehensive assessments of patients' physical abilities, functional limitations, and rehabilitation needs.
- Develop individualized treatment plans and goals in collaboration with patients, caregivers, and other members of the interdisciplinary team.
- Implement evidence-based physical therapy interventions and techniques to improve mobility, strength, balance, and coordination.
- Provide skilled therapeutic exercises, manual therapy, gait training, and other modalities to optimize functional outcomes.
- Educate patients and caregivers on safe and effective techniques for maximizing mobility, preventing injury, and promoting overall wellness.
- Monitor and evaluate patients' progress, adjusting treatment plans as needed to achieve desired goals and outcomes.
- Document all patient care activities, assessments, interventions, and outcomes accurately and timely in accordance with regulatory standards and agency policies.
- Communicate effectively with physicians, nurses, and other healthcare professionals to coordinate care, share information, and optimize treatment outcomes.
- Advocate for patients' rights and preferences, promoting a culture of respect, dignity, and autonomy.
- Participate in interdisciplinary team meetings, case conferences, and continuing education opportunities to enhance clinical skills and stay abreast of advances in physical therapy practice.
Job Types: Full-time, Part-time
Pay: $59.25 - $64.93 per visit
Expected hours: No less than 40 per week

Qualifications
Qualifications:
- Valid and current licensure as a Physical Therapist (PT) in the state of Michigan.
- 1-2 years of experience as a Physical Therapist, with previous experience in home health care preferred.
- Demonstrated proficiency in clinical skills, including assessment, treatment planning, and implementation of physical therapy interventions.
- Strong interpersonal and communication skills, with the ability to establish rapport with patients, families, and caregivers.
- Compassionate and empathetic attitude towards clients and their families, with a commitment to delivering person-centered care.
- Ability to work independently and collaboratively as part of a multidisciplinary team, prioritizing tasks and managing time effectively.
- Reliable transportation and willingness to travel to patients' homes within the designated service area.
- CPR certification and other relevant certifications may be required.
Salary : $59 - $65
