Summary/Objective
SafeHouse Outreach is seeking a compassionate and dedicated Mental Health Outreach Case Manager to provide case management and mental health support services to individuals experiencing homelessness and other underserved populations. This role focuses on addressing mental health needs, crisis intervention, and connecting individuals to essential behavioral health resources while empowering clients to achieve stability and self-sufficiency.
As a key member of SafeHouse Outreach’s Crisis Response Outreach Team, the Case Manager will conduct proactive street outreach, engage with individuals in acute mental health crises, provide immediate de-escalation, and offer referrals to appropriate services. This position requires a strong understanding of mental health conditions, trauma-informed care, harm reduction, and crisis intervention techniques, as well as the ability to navigate complex service systems.
Additionally, the Mental Health Outreach Case Manager will work to reduce barriers to mental health treatment, support clients in accessing psychiatric care and medication management, and advocate for policies and programs that promote mental wellness and long-term recovery. This role is highly collaborative, working with internal teams, healthcare providers, law enforcement, crisis stabilization units, and community organizations to ensure that individuals receive the support and services they need.

Key Responsibilities
Mental Health Case Management
- Conduct comprehensive mental health and psychosocial assessments to identify clients’ needs, strengths, and barriers.
- Develop and implement individualized mental health service plans in collaboration with clients, ensuring a trauma-informed and person-centered approach.
- Facilitate linkage to mental health services, including psychiatric care, counseling, peer support, and crisis stabilization programs.
- Provide short-term therapeutic interventions such as psychoeducation, coping skills training, and motivational interviewing to support clients in managing symptoms.
- Offer support for medication adherence, assisting clients in connecting with providers for medication management.
- Monitor client progress through regular check-ins, adjusting service plans as needed.
- Provide advocacy within healthcare and social service systems to ensure clients receive appropriate and equitable care.
- Maintain accurate clinical documentation, including progress notes, treatment plans, and referrals.
Crisis Response & Mental Health Outreach
- Conduct proactive street outreach to engage individuals experiencing mental health crises, psychosis, or emotional distress.
- Utilize de-escalation and harm reduction techniques to provide immediate support and stabilization.
- Assist in coordinating emergency psychiatric evaluations for individuals in acute distress, working closely with crisis teams, law enforcement, and mobile response units.
- Facilitate connections to crisis stabilization units, detox programs, and inpatient psychiatric facilities when appropriate.
- Offer emotional support and mental health education to clients, their families, and community members.
- Provide follow-up care to individuals post-crisis to ensure continued access to resources and services.
Collaboration & System Navigation
- Work closely with psychiatric providers, therapists, and behavioral health specialists to ensure continuity of care.
- Partner with local harm reduction programs, crisis intervention teams, and behavioral health organizations to enhance service delivery.
- Advocate for mental health policy changes and resource allocation within homeless services and healthcare systems.
- Participate in multi-disciplinary team meetings to discuss client progress and develop comprehensive care strategies.
- Engage in community education and awareness efforts, helping to reduce stigma surrounding mental illness and homelessness.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or a related field; Master’s degree preferred.
- Minimum of 2 years of experience in mental health case management, crisis intervention, or behavioral health outreach, preferably with individuals experiencing homelessness.
- Strong understanding of trauma-informed care, harm reduction, and de-escalation techniques.
- Familiarity with mental health diagnoses, substance use disorders, and co-occurring conditions.
- Knowledge of psychiatric services, community-based mental health resources, and crisis response systems.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Valid driver’s license and reliable transportation required for outreach activities.
Preferred Skills
- Licensed or credentialed professionals (LMSW, LCSW, LPC, etc.) are highly encouraged to apply.
- Bilingual
- Certification in Mental Health First Aid, Crisis Intervention, or Peer Support is preferred.
- Knowledge of local behavioral health systems and psychiatric crisis services.

Pay: $48,000.00 - $52,000.00 per hour
