What are the responsibilities and job description for the RN Case Manager - Hospice position at SAFREST?
SAFREST Resources is looking to hire a Registered Nurse with at least one year of Hospice, Home Care or Home Health Experience. This would be a Full time position in Fort Washington, PA. You will see patients in the following counties: Bucks (primarily the Lansdale area).
Job Summary
A Hospice RN works as part of an inter-disciplinary team to provide end of life care to hospice patients. Hospice serves patients with life limiting illness and applies appropriate interventions for the palliation of symptoms. The position requires a knowledgeable professional with exceptional clinical skills.
Responsibilities and Duties
Provide clinical services to hospice patients including scheduled and emergent visits to manage symptoms and provide support to patients and families. The candidate must be able to work well independently.
