Job Summary:
As the Clinical Manager, you will play a pivotal role in overseeing clinical operations, managing a team of healthcare professionals, and ensuring that patients receive the best possible care. This role will start with direct patient visits and clinical oversight, with a gradual transition into a more administrative and leadership position as the agency continues to expand.
Key Responsibilities:

Clinical Oversight: Lead and manage the clinical team to ensure exceptional patient care, adherence to agency policies, and compliance with healthcare regulations.

Patient Visits: Perform home visits, including initial assessments, care planning, and ongoing evaluations of patients' conditions.

Team Collaboration: Work closely with Registered Nurses, Certified Nursing Assistants, and other healthcare professionals to ensure seamless care coordination.

Case Management: Oversee complex patient cases, ensuring that each patient receives personalized, comprehensive care in line with their needs.

Regulatory Compliance: Ensure the agency's clinical practices comply with local, state, and federal regulations and industry standards.

OASIS Documentation: Complete OASIS assessments for patients and ensure accurate and timely documentation in compliance with Medicare regulations.

Business Development: Support agency growth efforts by participating in business development activities and helping to expand the patient base.

Administrative Transition: As the agency grows, transition into a more administrative and strategic leadership role, with responsibilities such as staff management, budgeting, and strategic planning.
Qualifications:

Education: BSN (Bachelor of Science in Nursing) required.

Experience: Minimum of 3 years of experience as an RN Case Manager, Field RN, or Home Health RN working in home care or home health.

OASIS Experience: Proven experience with OASIS documentation and assessments, required for compliance with Medicare and Medicaid guidelines.

Familiarity with Philadelphia: Must be familiar with the Philadelphia area, including local healthcare resources and community organizations.

Driver’s License & Vehicle: A valid driver’s license and reliable vehicle are required for patient visits.
